New Referral Form
Referral Form. Please review and download it at your convenience. Keep it
with you in light of any referrals you might be getting.
Getting referrals has become more difficult as NOA’s may only be changed
on annual dates. Therefore, more information about the NOA is needed
when referrals are taken. Please review with questions or concerns to me.

as usual with some great and unusual gifts (just ask Judy McKinney). For
those of you who didn’t get to participate, you missed a great time!
Day Service News
Most of you know that the Day Service packet had to be resubmitted. Linda
and Faith worked furiously and got all information together with the
multiple copies and down to the state house on November 30th. I attended
the provider training on the 7th just to ensure that we had everything and
believe it or not that have modified the packet (again). It turns out we have
seven more things (that I counted) that we will need to turn in. One of the
issues was my fault. I didn’t realize we had to provide meals which means
reformulating the budget to cover meal costs and the cost of a dietician. If
anyone knows of a good dietitian that wants to consult, have them send
their resume to our office!
Behavior Management News
As the company continues to wait on the state to respond to the appeals we
sent in, I did not feel comfortable making the behavior staff continue to work
without knowing what their salary was. I decided a leap of faith was
needed, so Jim and I configured what we felt will approximately be each
individuals salary if the appeals are accepted. Now, the behavior staff
know their salary for at least the next quarter. If you are a behaviorist and
haven’t reviewed a financial agreement yet, you will most likely do so
within the next week or so.
In addition, there is a high probability of a influx of behavior work within
the next few months. So, we are looking for another full time behavior
consultant. If you know of someone who needs work, have them send us a
resume.
You Never Know Who You Might Meet
Recently, one of our staff was casually enjoying her vacation, and guess who
she ran into?
Coming and Going
We have a number of new staff we have not mentioned yet in our
newsletter. Sarah Sharp was nice enough to move here from Tennessee and
will be providing Music Therapy in the Indy area. In addition, Ft.Wayne
now has two new staff from our company. Christine Stanford is our newest
behavior analyst. Kristin Dern is our newest Music Therapist.
We are sad to part ways with two staff persons. This includes Sarah Jurisa,
Music Therapist for the Indy area. Sarah will be returning to school to
further her education.
We are also sad to report that we will be losing Maria Cantrell this month.
Maria did an amazing job over the last year at keeping our office organized
and efficient. Maria was offered a job at Sycamore Services in Danville
(where she had worked previously).
Growth and Development Committee
The GDC had their first formal meeting on December 2, 2005. In attendance
was Linda Smith, Mandy McQueeny, Julie Edgell, Jim Sanders, Lisa Young,
and Danyl Epperheimer. Absent was Sharon Pape.
Future Growth was discussed in length, but the group felt that ensuring the
stability and well-being of our current employees were the first issues to be
addressed. Of specific importance ensuring that all staff feel supported and
a part of the company. We will be making some changes to help make this
happen. If you have specific ideas or recommendations, please email them

The group also discussed the problems of having clients “turn over” from
one therapist to another frequently.
